This is an increase from 2024's $174.70. The Part B premium typically increases annually based on healthcare costs.

Who Pays the Standard Premium?

You'll pay the standard $185.00 premium if your income from 2023 (the year used for 2025 determinations) is:

  • Single filers: $106,000 or less
  • Married filing jointly: $212,000 or less

About 93% of Medicare beneficiaries pay the standard premium.

Higher-Income Premiums (IRMAA)

If your income exceeds the thresholds, you'll pay more due to IRMAA (Income-Related Monthly Adjustment Amount):

How Part B Premiums Are Paid

Social Security Deduction

For most beneficiaries, Part B premiums are automatically deducted from Social Security benefits each month.

Direct Billing

If you don't receive Social Security benefits (or your benefits are too low), you'll receive a quarterly bill from Medicare. Payment options include:
  • Medicare Easy Pay (automatic bank withdrawal)
  • Credit/debit card online
  • Check by mail

With Medicare Advantage

Even with Medicare Advantage, you still pay the Part B premium. It's paid separately from any Medicare Advantage premium.

Part B Premium Assistance

Medicare Savings Programs

If you have limited income, state Medicare Savings Programs may pay your Part B premium:
  • QMB (Qualified Medicare Beneficiary): Pays Part B premium plus deductibles/coinsurance
  • SLMB (Specified Low-Income Medicare Beneficiary): Pays Part B premium only
  • QI (Qualifying Individual): Pays Part B premium only

Income Limits (2025, estimated)

  • QMB: Up to 100% FPL (~$15,060 single)
  • SLMB: Up to 120% FPL (~$18,072 single)
  • QI: Up to 135% FPL (~$20,331 single)

The Part B Deductible

In addition to the premium, Part B has an annual deductible: $257 in 2025.

After paying the deductible, you pay 20% coinsurance for most Part B services.
