2026 IRMAA Brackets

These are the full 2026 IRMAA brackets for every filing status. Find your 2024 income on the left, and the table shows your monthly Part B premium and Part D surcharge.

How to read these brackets

The standard 2026 Part B premium is $202.90 per month. If your income is above the first threshold, IRMAA replaces that standard premium with a higher one and adds a separate Part D surcharge. Remember that 2026 amounts are based on your 2024 Modified Adjusted Gross Income (MAGI) — adjusted gross income plus tax-exempt interest — because Medicare uses a two-year lookback.

What to do with this

If you are comfortably below the first threshold, you simply pay the standard premium. No. The Part D IRMAA surcharge is paid directly to Medicare, separate from the premium you pay your private Part D or Medicare Advantage drug plan.
